Occasionally osteoporotic fracture in elderly

- radiotherapy
- fatigue fracture in children
Don't present as isolated injury
- associated with pelvic fracture
Zone 1
- lateral to foramina
- neurologic injury from proximal migration & compression of L5 nerve root
