Disruption of the normal smooth motion of the scapulothoracic joint
- posterior scapula pain with overhead motion
1. Bony abnormalities
- osteochondromas
- prominence of the superior medial border of the scapula / Luschka tubercle
2. Soft tissue abnormalities
- bursitis / scar tissue
- muscular weakness
3. Masses
Inflammation and degeneration of the achilles tendon in the midsubstance 2 - 6 cm from insertion
Stenosing tenosynovitis of the first dorsal compartment of wrist
Most are middle aged women
Repetitive thumb movements
- abduction & extension
- combined with RD & UD movements
Any mechanical irritation
- foreign body
- prominent bony surface
- restricted fascial compartment
Full thickness tear (FTT)
- variable amount retraction from insertion
Partial thickness tear (PTT)
- incomplete
- bursal or articular sided
Mid-substance calcification of the rotator cuff
- part of a metaplasia secondary to hypoxia

Idiopathic inflammatory condition
- characterised by progressive shoulder pain & stiffness
- due to contracture of capsuloligamentous structures
- spontaneously resolves
2% incidence
- 40 - 60 years
- Women 2:1
Sedentary workers
- Non-dominant limb
Bilateral in 10 - 40%
Rotator cuff
- often deficient
Bone stock
- often deficient
Glenoid
- often posterior version
